3. Name, address, and phone number of your Veterinarian?
4. Why do you want to a Rat Terrier? What other animals do you own at this time?
5. What characteristics are you looking for in a Rat Terrier? (Temperament, size, color?)
6. Are you interested in a Male or a Female? If your selection is not available, would you accept the other?
7. Do you intend to breed or show this dog? Will you spay or neuter if for pet?
8. How many hours a day will the puppy be kept outside? How much time a day do you have to spend with a new dog? Do you have a fenced in yard or adequate enclosure?
9. Have you ever housetrained or crate trained a dog before? Are you willing to attend puppy obedience classes? How do you plan to give a Rat Terrier exercise?
10. What breeds of dogs have you owned before, How long, and what happened to them? Have you had to Euthanize an animal, if so why?
11. Name, age and relationship of other members in the same household? Referred by? How did you find us?
12. What do you expect the activity level of this breed to be?
